Menkes-Syndrom (MNK)

Menkes Krankheit ist eine seltene, X chromosomal rezessiv vererbte Störung des Kupferstoffwechsels, die durch pathogene Varianten im ATP7A-Gen verursacht wird und zu einer generalisierten Kupferdefizienz führt. Klinisch manifestiert sich die Erkrankung bereits im Neugeborenenalter mit prolongiertem Ikterus, Hypothermie, Hypoglykämie sowie Gedeihstörung und progressiver neuromotorischer Beeinträchtigung. Charakteristisch ist das spärliche, brüchige, stahlwollartige Haar (pili torti) sowie eine zunehmende Hypotonie, die später in Spastizität übergeht. Häufig entwickeln sich epileptische Anfälle, Bindegewebsmanifestationen und schwere neurodegenerative Veränderungen. Die Prävalenz wird in Europa und Japan auf etwa 1:300.000–1:360.000 geschätzt. Die Diagnostik stützt sich auf typische klinische Merkmale sowie erniedrigte Serumkupfer- und Ceruloplasminspiegel, die jedoch im Neugeborenenalter eingeschränkt aussagekräftig sind. Eine genetische Bestätigung durch die ATP7A-Analyse ist diagnostisch richtungsweisend. Kupfer Histidinat Therapie führt nur bei Beginn in den ersten Lebenstagen zu einer Reduktion der neurologischen Krankheitslast.

Die Kosten werden bei bestehender medizinischer Indikation über einen Überweisungsschein Typ 10 (EBM) abgerechnet. Humangenetische Leistungen sind nicht budgetrelevant. Für privatversicherte Patienten sowie private Kostenträger (Krankenhäuser etc.) können auf Wunsch entsprechende Kostenvoranschläge erstellt werden.
