Die zystische Fibrose (Mukoviszidose) ist eine autosomal rezessiv vererbte Multisystemerkrankung, die primär Epithelien von Atemwegen, exokrinem Pankreas, Darm, Leber und Schweißdrüsen betrifft. Klinisch dominieren eine chronisch progressive obstruktive Lungenerkrankung mit rezidivierenden bakteriellen Infektionen und Bronchiektasen, exokrine Pankreasinsuffizienz mit Maldigestion und Mangelernährung, gastrointestinale Komplikationen wie Mekoniumileus sowie männliche Infertilität durch bilaterale Aplasie der Ductus deferentes; pulmonale Manifestationen stellen die Hauptursache von Morbidität und Mortalität dar. Ursache ist eine Funktionsstörung des CFTR Proteins infolge biallelischer pathogener Varianten im CFTR Gen, das auf Chromosom 7q31.2 lokalisiert ist und einen ATP abhängigen Chloridkanal kodiert. In Europa beträgt die Prävalenz etwa 1:5.000 bei Geburt, mit geographischen Unterschieden.
Die Diagnostik stützt sich auf Neugeborenenscreening (erhöhtes immunreaktives Trypsinogen) und den Nachweis einer CFTR Funktionsstörung mittels Schweiß-Chlorid-Test (≥60 mmol/L) sowie den molekulargenetischen Nachweis krankheitsverursachender CFTR Varianten. Therapeutisch stehen eine multidisziplinäre symptomorientierte Behandlung (Atemphysiotherapie, Antibiotika, Pankreasenzym Substitution, Ernährungsunterstützung) sowie CFTR Modulatoren zur Verfügung, die bei geeigneten Genotypen den Krankheitsverlauf zunehmend positiv beeinflussen.

Die Kosten werden bei bestehender medizinischer Indikation über einen Überweisungsschein Typ 10 (EBM) abgerechnet. Humangenetische Leistungen sind nicht budgetrelevant.
